How to recover files from corrupted SD card?

I have a Lexar Professional 1000x UHS SDXC 128gb card that I've used to take photos from vacations I took over the past year. When I insert the card into my Macbook Pro, nothing happens / the card is not recognized. When I insert the card into my PC's card reader, I get a card error and am told it cannot be read. What's the best way to recover the files? Is it even possible or am I screwed? Is there any software anyone can recommend and should this be done via my Macbook or PC? Thanks all!

Let's work with your Mac first. You mentioned your Mac doesn't recognize SD card. This can result from disk corruption, incorrect configuration, loose connection, or even physical damage. 

To check whether it's a configuration issue, go to Disk Utility and see if the SD card shows up. If it does, it's a simple setting issue.
